Once you are scheduled, if you cannot make your appointment, please give the office 24 hours notice to avoid a broken appointment charge or reschedule the date and time.
You will first be asked to fill out new patient forms, if you have not already. Please bring a photo ID and insurance card(s), if applicable. For your convenience, new patient forms can be found below, completed in advance, and submitted prior to your appointment.
We participate with many major Dental Insurances. Please call the office to inquire if your insurance is on our list of participating companies.
While we do participate with a large number of insurance companies, if necessary or for more flexibility of payment, Care Credit is accepted for 6- or 12-month options. We also accept all major credit cards for your convenience.
